The non-recurring rate is waived if Internet Service
Provider (ISP) commits to twelve (12) months of
service.  Charge is not prorated based on number
of months of service actually provided.

MONTHLY-                                                                                             
Voice and Data Lines - $8.00 per line - For ISPs with greater
than 1,000 served lines. 
Voice and Data Lines - $16.00 per line - For ISPs with less than
1,000 served lines.

Data Only Lines - Not Available.                                                     

Serving Wire Center - Not Applicable - For ISPs with greater
than 1,000 served lines.
Serving Wire Center - $519.25 per center- For ISPs with less
than 1,000 served lines.  

Notice of Offering DSL (Wireline Broadband Internet Access Service) on a Permissively Detariffed Common Carrier Basis and Discontinuance of Offering on a Tariffed Basis Effective July 1, 2009

This is an informational Notice required by the Federal Communications Commission ("FCC").  Chazy & Westport Telephone Corporation ("Company") is required to notify its customers of its intent to discontinue providing DSL service on a tariffed basis.  A Notice was sent to all customers on May 20, 2009, and a Notice was filed with the FCC on May 21, 2009.

Beginning July 1, 2009, the Rates, Terms and Conditions for the Company's Wireline Broadband Internet Access Service ("WBIAS" also called "DSL") are available here on the Company's web site at www.chazyandwestport.com under "Products & Services" and will also be available for review at the Company's offices in Westport, New York.
